Directions:

• Go to "Provider Search" on the right hand side of screen below
• Choose "Physician/Specialist" (right side, midway down). This will launch a new window
• Go to the bottom right hand corner and click on "View Provider Directories" (very bottom, very right)
• Scroll Down to any state & Select any vicinity and then be sure to choose the plan that is in the ChoiceCare Network PPO
